To keep things simple, you can use your regular
LISCO username (assuming it's 4 or more characters long). Your
LISCO username is the first part of your LISCO e-mail address.
For example, "jdoe" is the username part of jdoe@lisco.com.
Your LISCOJET username can contain numbers
and letters, but NOT spaces, punctuation, or other symbols [such
as #, $, *, ?, etc.]. Your username MUST be 4 to 22 characters
long, all of which should be lowercase.
Note that when you later sign in to use LISCOJET
the first time, you will need to enter the username PLUS
the lisco.com. For example, jdoe would enter "jdoe@lisco.com"
as his LISCOJET username.

submitting this form I authorize LISCO to add this amount to
my monthly bill and to use my current LISCO account's payment
method, (by invoice, credit card, etc.) for payment.
